Blackstone was founded in 1798 and named after the Blackstone River, which runs through the town. The town was originally known as Blackstone Depot, as it was a stop on the Southside Railroad. The town was incorporated in 1874.
In the late 19th century, Blackstone was a major tobacco market, with warehouses and factories located in the town. The town was also home to a number of other businesses, including a cotton mill, a furniture factory, and a canning factory.

Blackstone is located at 37°7′17″N 78°7′17″W (37.1214, -78.1214). According to the United States Census Bureau, the town has a total area of 2.2 square miles (5.7 km2), all of it land.

The town is home to a number of agricultural businesses, including a poultry processing plant, a feed mill, and a number of farms. There were 1,364 households, out of which 28.2% had children under the age of 18 living with them, 43.2% were married couples living together, 17.7% had a female householder with no husband present, and 34.7% were non-families. 30.7% of all households were made up of individuals, and 15.2% had someone living alone who was 65 years of age or older. The average household size was 2.41 and the average family size was 3.00.
In the town, the population was spread out, with 24.7% under the age of 18, 8.2% from 18 to 24, 27.2% from 25 to 44, 22.3% from 45 to 64, and 17.6% who were 65 years of age or older. The median age was 38 years. For every 100 females, there were 87.2 males. For every 100 females age 18 and over, there were 81.3 males.

The per capita income for the town was $15,945.
