The county seat is Medicine Lodge, and the county was named after Thomas Barber, a Union soldier who died in the Civil War. As of the 2010 census, the population of Barber County was 4,861, making it the second-least populous county in Kansas.

The county is drained by the Medicine Lodge River, which flows through the county from north to south.
