Middletown was founded in 1802 by a group of settlers from New Jersey. The city was named after Middletown, New Jersey, the hometown of one of the settlers. The city was incorporated in 1833 and became the county seat of Butler County in 1848.

The city was also home to the Middletown Works, a large steel mill that was operated by the Armco Steel Corporation. The Middletown Works was the largest steel mill in the world at the time of its closure in 1986.
